Gutierrez Family Medicine, formerly located at 6707 N 19th Ave, Suite 104, Phoenix, AZ 85015, closed in August 2026. Dr. Gutierrez will continue caring for her patients through HealthyU Clinics and will begin seeing patients at her new location:
